Medical Accommodation Policy

The University recognizes that a student’s ability to meet his/her academic responsibilities may, on occasion, be impaired by medical illness. Illness may be acute (short term), or it may be chronic (long term), or chronic with acute episodes. The University further recognizes that medical situations are deeply personal and respects the need for privacy and confidentiality in these matters. However, in order to ensure fairness and consistency for all students, academic accommodation for work representing 10% or more of the student’s overall grade in the course shall be granted only in those cases where there is documentation indicating that the student was seriously affected by illness and could not reasonably be expected to meet his/her academic responsibilities.You can view the information about Medical Accommodation in the Academic Handbook.

How to Request Academic Accommodation

STEP 1: Review course OWL page or Course Outline for notes regarding accommodation. Academic Advisors can recommend accommodation for course work that is worth 10% or more of your final grade.

STEP 2: Obtain the appropriate documentation to support your request. You may consult the table below for more information. Click on 'Types of Documentation'.

STEP 3: Complete an Accommodation Request Form.

STEP 4: Submit your accommodation request and paperwork to the Student Support Services Office (W45) or by email to huronsss@uwo.ca. An automated message will be sent to you and the specified instructor(s) making the recommendation for accommodation.

The student is responsible for contacting their instructor to organize the submission of accommodated work or the date for a make-up test. Please contact your instructor as soon as possible regarding your accommodation.

Note: if you are a student registered at Services for Students with Disabilities for Accommodated Exams, please complete Makeup Exam Request form. You will need to obtain signatures from your Academic Advisor and Professor and return it to Student Central (Room 1120 Western Student Services Building).

If your reason for requesting accommodation is not listed above, you may speak directly with your Instructor. Any accommodation offered is at the Instructor's discretion and may not be granted. Family vacations, flights and travel arrangements are not grounds for accommodation under Academic Advising.

    • 5 or more exams/mid-year tests scheduled within71 hours.

  • Requests for Accommodation for Final Exams

    1. Please complete all the steps required for Requests for Accommodations for Course Evaluations Worth 10% or More.

    2. Once your Accommodation Request has been approved, you can obtain a Recommendation for a Special Exam form from your Academic Advisor.

    3. Obtain the required signatures on the Recommendation for a Special Exam form and return to Student Support Services (W45). 

    Note: if you do not return the signed Recommendation for Special Exam form, there will be no make-up exam released for you to write.
